Surveys

As part of our ongoing assessment, we are asking people to participate in surveys to document current patterns of behavior and training needs in our region.

Cultural Competency Self-Assessment:  assesses the training needs that may be available within our region

Young Adult (18-34) Survey:  assesses the perceptions of young adults about consumption of alcohol, tobacco, and other drugs  

Educators (K-12) Survey:  assesses the perceptions of K-12 educators about the consumption of alcohol, tobacco, and other drugs

Youth Survey:  assesses the perceptions of youth about the consumption of alcohol, tobacco, and other drugs

Parents Survey:  assesses the perceptions of parents about the consumption of alcohol, tobacco, and other drugs
